Myers Family Dentistry

New Patients

At this time unfortunately we are not taking new patients-  Please check back with us soon.

During your first visit to our office, we take x-rays in order to see the whole picture of your mouth. If you have x-rays that have been taken recently, we can request them from your previous office. Dr. Myers will provide a comprehensive exam where he examines every tooth and the entire mouth- checking for any cavities, gum disease, or any other issues. From this information, we will discuss options of treatment with you and develop a treatment plan that best meets your needs.

If you are in pain and require immediate attention, we will perform a limited exam to determine the cause of your discomfort and do everything that we can to get you out of pain. Any information that you can provide to the doctor about the pain (including the frequency, duration, and initiating factors) will aid in establishing a diagnosis. We will also stabilize your mouth for any further treatment that may be needed.

We ask that you bring your dental insurance information, a list of current medications you are taking, and any relevant personal medical information so that we can safely treat you.

See attached for the medical history and insurance information forms. Forms are available online to be printed and completed prior to your appointment or they can be picked up at our office.
